Outpatient appointments survey

Feedback from patients and visitors tells us that most people are able to find their way to outpatient appointments quite easily but some find it difficult for a variety of reasons.

We are conducting a short survey as we need to understand what makes it difficult so that we can take action to improve things where needed.

All feedback is welcomed, however we are particularly looking for feedback from patients and carers who have experienced difficulty in finding where they needed to go for their appointment.

We are also keen to hear from those who may have found that necessary information was not communicated in a way that met their needs.

Feedback is collected anonymously, treated confidentially and will not affect your ongoing treatment in any way.
